Membership Application / Renewal

Please complete the membership application / renewal process outlined below.

 

Step 1. Make your payment

• by credit card or electronic funds transfer (EFT) (information entered below must match the official application / renewal form)

• send an etransfer to info@perthseniors.ca (email and name must match what you enter on the application form unless you are paying for your spouse, in which case you pay $80 and indicate both of your names in the etransfer message)
• by cheque or cash (enclose with printed application form)

 

Step 2. Complete Application / Renewal Form

Complete the official application / renewal form. If you pay by credit card, EFT or etransfer, you can email the form to info@perthseniors.ca. If you pay by cash or cheque, you must print the form and drop it off (into the mail slot) or mail it to the office at 125 Gore St E, Perth, ON K7H 1J6.
